Our medical director is unique in that she is available 24 hr/day. She is the physician who coordinates the team to create an individual plan of care tailored to each individual patient’s needs, based on their symptoms and wishes. 

Our hospice nurse is available to our patients 24/7 in case of a crisis. She visits regularly to assess, organize, and implement a plan of care based on the patient’s individual wishes. Hospice nurses are specially trained to treat symptoms and increase comfort. 

Our social worker helps assess a patient’s and family’s psychosocial and financial needs to make sure they are met. The social worker can also provide direct counseling and arrange referrals to community agencies as necessary.

In treating the whole person - body, mind and spirit, our non-denominational chaplain assesses patient’s and family’s spiritual needs. They are also helpful in providing bereavement counseling for a minimum of 13 months after the death of a loved one.

​Our CNAs assist our patients and family with personal care needs, such as bathing, dressing and feeding.
